1. Tell the practice
Email info@cortextelehealth.com.au with "Complaint" in the subject. Include your name, the date of the appointment if there was one, and what you would like to happen. Please don't send a long medical history if a shorter note will do. We will acknowledge the email and look into it.
TODO: add a phone number and a postal address for complaints once Brett confirms which contact details to publish, and name who handles the complaint if it is about Brett himself.
2. Registration and conduct
Concerns about a Nurse Practitioner's conduct, performance or health can be raised with the Australian Health Practitioner Regulation Agency (Ahpra), which works with the Nursing and Midwifery Board of Australia.

3. The health service
A complaint about the health service, as distinct from a practitioner's registration, goes to the health complaints body in your state or territory. The practice materials point to Wagga Wagga, New South Wales, so many patients will use the Health Care Complaints Commission. If you live elsewhere, use the body in your own state or territory. Ahpra lists them.

4. Privacy
Privacy complaints start with us, then the Office of the Australian Information Commissioner if you wish: oaic.gov.au, 1300 363 992. The privacy policy has more detail and is also a draft.
